Clients often ask me if it is possible to integrate third-party form builder applications with Microsoft Dynamics 365 and the broader Power Platform. The short answer to that question is yes. However, there are a number of considerations to be made when choosing which is the right form builder application for your deployment.
Evaluation of Third-Party Form Builder applications
Some of the factors that may affect your decision to choose the right third party form builder include:

During my evaluation there were a number of notable form builder applications that stood out. In no order of preference, they are:
The good news is that when it comes to integration with Microsoft, there is a Power Automate certified connector for Mailchimp. Documentation for this connector with available actions and triggers is located at https://learn.microsoft.com/en-gb/connectors/mailchimp/
Snapforms is a lightweight form designer/builder and has a third party non-certified connector that can be used with Power Automate. A provided JSON file is provided so that you can may integrate the solution with Power Automate as a custom connector or as an Azure Logic App. All common actions such as Upon Submit, download response payload, download as PDF are available.
Snapforms offer a trial for users, and their pricing ranges from $39aud/month for the standard edition through to $129aud/month for their Professional edition. Note that API web services support is only offered on their Professional tier.
Fortunately, there is a certified Power Automate connector available that offers a series of actions and triggers that allow you to extract responses from the platform. They are described at the following documentation site:
https://learn.microsoft.com/en-us/connectors/surveymonkey/
There is also a free trial available, and licensing starts from $23aud/month through to $75aud/month with an enterprise option available upon request.
JotForm does have an certified Power Automate connector for both their enterprise and non-enterprise customers. The documentation is available from the following website:
https://learn.microsoft.com/en-us/connectors/jotformenterprise/
Additionally there is a free trial available, with pricing starting at $34/month through to $99/month with enterprise licensing available upon request.
While there is no certified connector for Power Automate, they do offer extensive web services that enable a developer to build custom connectors for Logic Apps and Power Automate for practically any given scenario.
There is a trial available for CustomerXM, and price is available upon request.
The above is by no means an exhaustive list of products that work well together with Dynamics 365 and the Power Platform, and merely serve as an example of what is out in the market and the capabilities which they support. Note, I am no way affiliated with any of the above companies, and merely compiled this list based on my independent research of available products.
